What procedures require authorization? The outpatient procedures requiring authorization include the following:
- Nuclear Stress Tests
- Echo Stress Tests
- Echocardiograms
- Coronary CT Angiograms
Hospital admissions for the following procedures require authorization:
- Diagnostic Left Heart Catheterizations
- Pacemakers
- Implantable Cardioverter Defibrillators
- Biventricular Pacemakers
How is authorization obtained? The physician managing your treatment plan must obtain the authorizations necessary for your exams.
What information is required during the authorization process? An on-line procedure request form (link) is available. This form details the information that may be required to proceed with the authorization process.
How do I know when a procedure has been authorized? You will be notified when an authorization is issued. Additionally, you may initiate an on-line Authorization Lookup (link) to obtain an authorization number for approved procedures.
What if my exam does not get approved? If the requested exam is denied, CareCore will notify both you and your patient with the reason for the denial and detailed Appeal instructions.
