CareCore National works with clients to provide a detailed assessment of each professional imaging practice, ensuring that the Health Plan's contracted centers meet professional and technical performance quality standards.

For your convenience, below you will find the required forms in PDF file for participation in the CareCore National networks. We will provide a detailed report on each professional practice and how it performs based on accepted standards.

For providers requesting participation in CareCore National's Diagnostic Imaging Network, or for those currently participating providers wishing to remain as participating providers, the following Minimum Equipment Standards are in place, effective June 1, 2006.
